Parent Expectations

All parents and guardians are required to agree to the Parent Code of Conduct, Spectator Removal Policy, and Discipline Escalation Policy during registration. These policies ensure a safe and respectful environment for all participants.

Participation Waiver & Release of Liability I understand that participation in youth football and cheerleading involves physical contact, strenuous activity, and inherent risks, including the risk of serious injury. I voluntarily assume all risks associated with my child’s participation. I hereby release, waive, and discharge [The Dolton Bears], its officers, directors, coaches, volunteers, sponsors, and affiliates from any and all claims, demands, or causes of action arising from participation, including those resulting from ordinary negligence, to the fullest extent permitted by Illinois law.
MEDICAL AUTHORIZATION In the event of an emergency and if I cannot be reached, I authorize The Dolton Bears and its representatives to obtain emergency medical treatment for my child. I understand that I am financially responsible for any medical care or treatment provided.
PHOTO & MEDIA RELEASE I grant permission for The Dolton Bears to photograph or record my child during practices, games, or events and to use such images or recordings for promotional, educational, or marketing purposes without compensation.
PAYMENT AGREEMENT & INSTALLMENT PLAN ACKNOWLEDGMENT I understand that by registering my child, I am responsible for paying the full registration fee. If I select an installment payment plan, I agree to pay today’s required deposit and understand that remaining payments will be invoiced separately through Square. Failure to complete payments may result in suspension or removal from participation.
NO REFUND POLICY ACKNOWLEDGMENT I understand and agree that all registration fees are non-refundable, regardless of injury, illness, withdrawal, dismissal, schedule changes, or failure to participate. This policy applies to both full and installment payments.
ACKNOWLEDGMENT OF ACCURATE INFORMATION I certify that all information provided in this registration is accurate and complete. I understand that providing false or incomplete information may result in denial of participation.
Parent Code of Conduct Agreement I have read and agree to abide by the Parent Code of Conduct established by The Dolton Bears, and I understand that violations may result in disciplinary action, including removal from participation without refund.
